
Synchronized cardioversion is a procedure that delivers a controlled electrical shock precisely timed to the R wave of the heart's rhythm, avoiding the vulnerable period that could trigger a more dangerous arrhythmia. It is used to convert organized but unstable rhythms back to normal.
It is indicated for patients who are unstable due to tachyarrhythmias such as atrial fibrillation with a rapid response, atrial flutter, supraventricular tachycardia, or stable ventricular tachycardia with a pulse. This differs from defibrillation, which uses an unsynchronized shock for pulseless, chaotic rhythms. Correct energy selection, sedation when possible, and proper synchronization are key elements of the ACLS approach.
