
Pulseless Electrical Activity (PEA) is a form of cardiac arrest in which the heart's electrical system continues to generate organized activity visible on a monitor, but the heart fails to produce a detectable pulse or effective blood circulation. In other words, the electrical signal is present, yet the mechanical pumping is absent or insufficient.
PEA is a non-shockable rhythm, so defibrillation is not used. Treatment relies on immediate high-quality CPR, epinephrine, and an urgent, focused search for reversible causes — the Hs and Ts, including hypoxia, hypovolemia, tension pneumothorax, cardiac tamponade, and toxins. Identifying and correcting the underlying cause offers the best chance of restoring circulation.
